Hair Shedding on a GLP-1: Biotin, Collagen, and the Evidence

Hair shedding is one of the more upsetting things people notice after starting a GLP-1 medication. The short answer: shedding after fast weight loss is common and usually temporary. Biotin helps only people who are low in biotin. Collagen has mixed evidence for skin and little for hair.

Here is why shedding happens, what the supplement evidence shows, one lab-test warning, and how to compare cost per serving.

Why hair sheds after weight loss

  • Shedding after a big change is a known pattern. Heavy shedding, called telogen effluvium, can follow a stressor such as losing 20 pounds or more. It tends to show up a few months later (American Academy of Dermatology).
  • It usually settles. Hair tends to return to its normal fullness within 6 to 9 months (American Academy of Dermatology). Losing 50 to 100 hairs a day is normal.
  • It shows up in GLP-1 data. In semaglutide 2.4 mg weight trials, 3% of people reported hair loss, against 1% on placebo (Semaglutide 2.4 mg label 2025). A large health-record study also found more shedding diagnoses in GLP-1 users, likely linked to fast weight loss (Vidal et al. 2026). That study shows an association, not a cause.

Biotin: helps only if you are low

  • The evidence is thin for most people. A review found every reported case of biotin helping hair or nails involved someone with a deficiency or an underlying condition. It found no good evidence for people with normal biotin levels (Patel et al. 2017).
  • Hair products use very high doses. The adequate intake for adults is 30 micrograms a day. Hair products often contain far more (Patel et al. 2017).

The biotin lab-test warning

  • Biotin can distort blood tests. The FDA warns that biotin supplements can cause incorrect results on some lab tests, and the error may go unnoticed (FDA 2022).
  • That includes a heart test. Some troponin tests, used to diagnose heart attacks, can read falsely low (FDA 2022).
  • What to do: tell your care team you take biotin before any blood work. Many hair, skin, and nail products contain it, so check the label.

Collagen: mixed evidence for skin

  • Pooled trials look positive. A 2021 meta-analysis of 19 trials found collagen improved skin hydration and elasticity over about 90 days (de Miranda et al. 2021).
  • The benefit fades in independent trials. A 2025 meta-analysis of 23 trials found the benefit disappeared in trials not funded by industry and in higher-quality trials (Myung & Park 2025).
  • Hair is barely studied. Both analyses measured skin, not hair.

How to compare hair and skin products

  • Check the biotin dose. If a product has biotin, note the amount and remember the lab-test warning.
  • Check grams of collagen per serving. Trial doses were measured in grams, not milligrams. Capsule products often need many capsules per serving.
  • Count the units. Gummy and capsule servings are often two or more units. Our gummy guide shows why the count on the front misleads.

When to talk to your prescriber

Shedding that lasts longer than about 6 months, patches of hair loss, or scalp changes are reasons to ask about it. Low iron or thyroid problems can also cause shedding, and a blood test can check.

Check with your care team first if:

  • Pregnant, trying to conceive, or breastfeeding. Ask your prescriber before starting any new supplement.
  • Type 1 diabetes, or taking insulin or a sulfonylurea. Products sold for blood sugar support, such as berberine or chromium, may add to the effect of those medicines. Ask before combining them.
  • A history of an eating disorder. Appetite and weight products can make recovery harder. Talk with your care team first.
  • Kidney disease. Electrolyte, protein, and creatine products change what your kidneys have to handle. Ask about your limits.
  • Other prescriptions. Fiber can slow how some medicines are absorbed. Ask your pharmacist how to space doses.
